Abstrakt: [cze][eng] Diplomová práce má za cíl v teoretické části popsat problematiku funkčních poruch horního trávicího traktu (GIT) s důrazem na gastroezofageální refluxní nemoc (GERD). Teoretická část popisuje anatomii, patofyziologii s etiologií GERD a možnosti léčby. V praktické části je u vybraných 15 probandů s GERD měřena posturální reaktibilita bránice pomocí vysokorozlišovací manometrie v různých posturálních situacích: leh na zádech s trojflexí dolních končetin, stoj, stoj na podložce AIREX, ve stoje jsme použili zátěž o hmotnosti 3, 6 a 9 kg v těžišti a mimo těžiště. Pacienti také vyplnili dotazník Health-Related Quality of Life pro GERD (GERD-HRQL). Měření potvrdila zvýšení klidového tlaku LES ve všech pozicích mimo stoj, což koreluje s největším výskytem refluxních epizod ve stoji. Výsledky byly porovnány s kontrolní zdravou skupinou. Tendence v aktivaci krurální části bránice byla u obou skupin stejná. U pacientů s GERD byla zjištěna horší koordinace, opožděná a neplynulá aktivace krurální části bránice. Nejvýznamnější zvýšení tlaku LES u pacientů s GERD nastalo v posturální pozici trojflexe DKK nad podložku a vestoje při držení břemene o hmotnosti 9 kg mimo těžiště. V pozici trojflexe DKK vzrostl tlak z průměrných 17,27 mm Hg na 36,53 mm Hg se statistickou významností p = 0,0012. Rehabilitace u...The diploma thesis aims to describe the functional disorders of the upper gastrointestinal tract with an emphasis on gastroesophageal reflux disease. The theoretical part describes the anatomy, pathophysiology, etiology of GERD and treatment options. In the practical part is measured postural reactivity of the diaphragm in the selected 15 probands with GERD. We measured the postural reactivity of the diaphragm using high resolution manometry in various postural situations: lying on the back with flexion of lower limbs above the surface, standing and standing on the AIREX mat, for standing patients, we used weights of 3,6 and 9 kilos inside and outside the center of gravity. Patients also completed the GERD Health-Related Quality of Life questionnaire. The measurements confirmed an increase in resting pressure in all positions except standing, which correlated with the highest occurrence of reflux episodes in standing. The results were compared with a healthy control group. The activation of the crural part of the diaphragm was the same in both groups. We found in patients with GERD decreased coordination, delayed and influent activation of the crural diaphragm.
